Actually we do know what the Shred, Broken flute, and Tattered cape are for - they are for a quest that is currently broken it will remain so until "a later build" (weather that means a patch or an expansion, your guess is as good as mine)

I have made a Mesmer, Elementalist, Monk, Necromancer, and Ranger - every single one of them got the shred without a problem, I also didnt hunt for the flowers, I just did every quest and gave her the flowers as I came across them. It is completley random.
